Effective March 22, 2023, Dr. Martin Webb will no longer be working with Carolina Kidney Associates.
The physicians and staff are committed to providing exceptional care to our patients. If Dr. Webb was your personal MD, Carolina Kidney is prepared to assign you a new provider and your care will not be interrupted. If you have an appointment with Dr. Webb in the next 75 days, you will receive a phone call to let you know if the date of your appointment will need to change and you will be given the name of your new provider.
If, however, you would prefer to seek medical care elsewhere, you are free to do so. You may request a copy of your records or you may request them to be transferred directly to another office. For either of these options, we will need a signed authorization to transfer records. A Medical Records Release Form is available on our website. You can find the form here.
